Using Service Plans

A service plan is one of the most important documents a direct support professional, or DSP, uses. A service plan documents what services a person needs and how you can help them live their best life. A well-developed plan is a valuable tool that can help you do your job well. It is also an agreement between you and the person you support. This course will teach you more about using a service plan and putting it into practice.

This course includes a sample service plan for a fictional woman named Kay Klayton. The service plans your agency uses may look very different from this sample. Kay’s sample plan is intended to illustrate the concepts explained in this course. It does not include all of the details a typical service plan might include.

The goal of this course is to provide direct support professionals in IDD service settings with an overview of how to use service plans.
